目的 通过影像归档和通信系统(PACS)测量成人下颈椎关节突关节解剖参数,并探讨其临床意义。方法 选择2014年9月-2015年2月本院63例成年门诊患者,行颈椎CT检查,所得数据传输至PACS,然后在AquariusNETViewer (V4.4.6.80)窗口界面手工进行颈椎关节突关节各参数的测量分析。所得数据纳入统计学分析,比较同一关节突关节左右侧的差异以及男女间的差异。结果 成年人下颈椎同一关节突关节的左右两侧横径、矢状径及外斜径差异无统计学意义,成年人下颈椎双侧关节突关节基本对称。男性下颈椎不同关节突关节面的矢状径为11~13 mm,外斜径为11~12 mm;女性矢状径为10~12 mm,外斜径为10~11 mm,男性下颈椎关节突关节明显大于女性。结论 PACS测量成人下关节突关节解剖参数具有较高的一致性,对颈椎后路置钉融合手术中侧块螺钉的选择具有参考意义。按Magerl或Anderson方法置入下颈椎侧块螺钉时,男性选择长12~14 mm的螺钉是安全的,而对于女性则建议选择长12 mm的侧块螺钉。 |

Objective To measure the zygapophyseal joints anatomic parameters of the lower cervical spines in adults by picture archiving and communication system (PACS) and explore its clinical significance.Methods Sixty-three adults were included in this study, all of whom received CT examination on the cervical spine, and the relative data of cervical zygapophyseal joints were transmitted manually to PACS.Then in AquariusNET Viewer (V4.4.6.80) the parameters were measured manually.Statistical analysis was carried out subsequently, then the mean and standard deviation were calculated and compared between both sides and between male and female subjects.Results No statistical differences were found in transverse, sagittal and oblique diameter respectively between both sides of cervical zygapophyseal joints in adults.Bilateral zygapophyseal joints of the lower cervical spine in adults are symmetrical on the whole.The sagittal diameter was about 11-13 mm, and oblique diameter 11-12 mm in male adults, while 10-12 and 10-11 mm respectively in female ones.Zygapophyseal joints in the male were bigger than those in the female.Conclusion Measurement of the zygapophyseal joints anatomic parameters of the lower cervical spines in adults by PACS have high consistency, and it has reference value for the selection of the lateral mass screws in the posterior cervical fusion surgery.According to the Magerl or Anderson technique for the insertion of lateral mass screws, the length of 12-14 mm is safe for the male, and 12 mm for the female. |
