目的 探讨采用MRI测定的脊柱骨折患者双侧多裂肌和竖脊肌横截面积(CSA)与腰背部疼痛的相关性。 方法 回顾性分析2019年4月—2022年4月接受非手术治疗的100例T12和/或L1压缩性骨折患者的临床资料(骨折组),另选取同期体检的100名健康成人作为对照组。通过MRI T1WI像、T2WI像及Image J2x软件测量2组的T12/L1、L1/L2水平双侧多裂肌和竖脊肌相对横截面积(RCSA),以疼痛视觉模拟量表(VAS)评分4分为界比较腰背部不同疼痛程度的骨折患者多裂肌与竖脊肌总RCSA的差异,采用多元线性回归分析年龄、性别、VAS评分与双侧多裂肌与竖脊肌RCSA的相关性。 结果 骨折组双侧多裂肌及竖脊肌总RCSA低于对照组,差异均有统计学意义(P < 0.05)。VAS评分≥4分组患者的双侧多裂肌及竖脊肌总RCSA较VAS评分< 4分组患者明显下降,差异均有统计学意义(P < 0.05)。多元线性回归分析结果显示,年龄、VAS评分与双侧多裂肌及竖脊肌总RCSA呈负相关(P < 0.05)。 结论 脊柱骨折患者可能存在椎旁多裂肌和竖脊肌萎缩,其萎缩程度与腰背部疼痛存在相关性,且年龄对双侧多裂肌和竖脊肌萎缩也有一定影响。 |

Objective To investigate the correlation of the cross-sectional area(CSA) of the bilateral multifidus and the erector spinae measured by MRI with low back pain in spinal fracture patients. Methods The clinical data of 100 patients with T12 and/or L1 compression fractures who received non-surgical treatment from April 2019 to April 2022 were retrospectively analyzed(fracture group),and 100 healthy adults who underwent physical examination during the same period were selected as the control group. MRI T1WI,T2WI and Image J2x software were used to measure the total relative cross-sectional area(RCSA) of the bilateral multifidus and the erector spinae at T12/L1 and L1/L2 levels in the 2 groups. The differences of the bilateral multifidus and the erector spinae total RCSA between patients with different pain intensity in fracture group were compared,and a 4 points of visual analogue scale(VAS) score were used as the boundary. Multiple linear regression was used to analyze the correlation between age,sex,VAS score and bilateral multifidus and erector spinae total RCSA. Results The bilateral multifidus and erector spinae total RCSA in the fracture group was lower than that in the control group,with a statistical significance(P < 0.05). The bilateral multifidus and erector spinae total RCSA in patients with VAS score ≥4 was significantly lower than that in patients with VAS score < 4,and the difference was statistically significant(P < 0.05). The results of multiple linear regression analysis showed that age and VAS score were negatively correlated with the bilateral multifidus and erector spinae total RCSA(P < 0.05). Conclusion Para spinal multifidus and erector spinae atrophy may be present in patients with spinal fracture,and the degree of atrophy is correlated with low back pain,and age also has a certain influence on bilateral multifidus and erector spinae atrophy. |
