术后反应性疼痛与引流液炎症因子相关性

摘要: 目的 探讨腰椎术后反应性疼痛与引流液中炎症因子的相关性。方法 2012年3月-7月,40例单节段腰椎椎间盘突出患者在本院行单节段腰椎后路椎板减压、椎间盘切除、椎间植骨融合内固定术。其中出现术后非切口部位的腰臀部或下肢疼痛13例。于术后1~3 d连续记录出现非切口部位疼痛的视觉模拟量表(VAS)评分。术后1~3 d连续留取伤口引流液,检测伤口局部炎症因子白介素-1β(IL-1β)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平及外周血炎症因子水平;术后1~3 d连续检测红细胞沉降率(ESR)、C反应蛋白(CRP)。通过统计学分析,探讨炎症因子与疼痛程度、ESR及CRP的相关性。结果 引流液IL-1β,IL-6,TNF-α水平与术后疼痛VAS评分呈正相关,而与血液中炎症因子水平无相关性,与ESR,CRP无相关性。结论 腰椎术后反应性疼痛与术后引流液中IL-1β,IL-6,TNF-α水平显著相关,提示疼痛与术后炎症因子的局部增加有密切的关系。

Correlation between lumbar spinal reactive pain and inflammatory factors in drainage fluid after lumbar spinal surgery

Abstract: Objective To explore the correlation between lumbar spinal reactive pain and inflammatory factors in the drainage fluid after lumbar spinal surgery. Methods From March 2012 to July 2012,40 patients with single-level lumbar disc herniation were treated by posterior lumbar decompression,diskectomy,interbody fusion and internal fixation. Reactive pain of the low lumbar or lower limb occurred in 13 cases at post-operation. Visual analogue scale(VAS) score was recorded for low back pain at 1-3 d post-operation. The interleukin-1β(IL-1β),interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) were tested in wound drainage fluid and inflammatory factors in peripheral blood at 1-3 d post-operation continuously. Erythrocyte sedimentation rate(ESR) and C-reaction protein(CRP) were assayed tested continuously at 1-3 d post-operation. The correlations of inflammatory factors with pain intensity,ESR and CRP were explored by statistical analysis. Results The concentrations of IL-1β,IL-6 and TNF-α in drainage fluid at 2- 3 d postoperative were positively correlated with VAS score(P<0.05),but did not correlate with the inflammatory factor level,ESR and CRP in the peripheral blood(P>0.05). Conclusion The reactive pain after lumbar surgery is correlated with the concentration of IL-1β,IL-6 and TNF-α in drainage fluid,indicating that the pain has a close relationship with local increase of inflammatory factors after surgery.

有研究证实炎症因子在椎间盘病变、下腰痛及放射痛中起了重要的作用[1, 2],退变性椎间盘内的炎症因子包括肿瘤坏死因子-α(TNF-α)[3]、白介素-1β(IL-1β)、白介素-6(IL-6)[4, 5, 6]。髓核、纤维环的破裂,可释放出炎症因子,引起无菌性炎症反应,感觉神经分布在纤维环最外层[7, 8],容易受到局部环境改变的影响,其中部分对于炎症反应敏感或较重的患者,腰椎术后纤维环的破坏,髓核内炎症因子可直接刺激神经根引起腰部疼痛,同时引起肢体疼痛或神经功能障碍。本课题组在临床实践中发现,部分患者在椎间盘切除并神经彻底减压的基础上仍会出现短期的非切口部位的下腰痛、坐骨神经痛等症状,经过激素或非甾体抗炎药的使用,症状明显减轻或逐步缓解。这些症状无法用神经压迫或损伤来解释,但给患者造成很大的痛苦,容易使患者及医生对术后的效果产生疑虑。本课题组推断这种疼痛可能是术后炎症因子局部释放增加引起的炎症反应性疼痛。

1 资料与方法 1.1 一般资料

2012 年3 月—7 月,40 例单节段腰椎椎间盘突出病例,经非手术治疗无效,在本院接受单节段腰椎后路椎板减压、椎间盘切除、椎间植骨融合内固定术,其中男18 例,女22 例;年龄46 ~ 65 岁,平均54 岁。排除腰椎结核、外伤、肿瘤、感染、有腰椎手术史及6 个月内使用过糖皮质激素的患者。所有手术均由高年资脊柱外科医生完成,内固定器械均由强生公司提供。

所有患者术中无硬脊膜破裂及脑脊液漏出现。术后伤口留置负压引流管3 d,伤口引流液均呈血性。在拔出引流管时记录伤口总引流量。所有患者未出现伤口深、浅部感染。术中止血彻底,避免对标本中炎症因子浓度产生影响,术后不使用激素,术后伤口疼痛剧烈时按需给予临时止痛药,术后3 d内不给予非甾体抗炎药。在40 例患者中有13 例出现非切口部位的腰臀部或下肢疼痛。于术后1 d、2 d及3 d 采用视觉模拟量表(VAS)[9]评价非切口部位疼痛程度,同时术后1 ~ 3 d 每日收集引流液并抽取外周血。为避免VAS 评分及标本收集误差,VAS 评分采取第三者单盲法。研究征得患者知情同意,且经过医院伦理委员会批准。

1.2 标本收集及保存 1.2.1 引流液收集

为避免血液中炎症因子的影响,以患者回病房为时间点,术后当日引流1 h 后去除引流液,继续引流10 mL 时收集,术后次日以相同时间点去除残留引流液,并更换引流袋后继续引流到10 mL时收集引流液,术后第三日采取与术后次日相同的收集方法,同时统计总引流量。标本收集后立即采用差速离心法,即分2 次离心,均离心15 min,首次1 500 r/min,再以3 500 r/min 离心,离心半径11.5 cm。分别取上清液置于1 mL EP 管,-80℃冰箱冻存。

1.2.2 血液收集

每日于清晨空腹收集术后1 ~ 3 d 血液标本,标本离心及保存方法同上,同时检测血液中C 反应蛋白(CRP)、红细胞沉降率(ESR)。

1.3 炎症因子检测

炎症因子IL-1β、IL-6、TNF-α 采用酶联免疫吸附实验(ELISA)进行检测,试剂盒由北京凯诺春天生物科技有限公司提供,检测步骤根据试剂盒说明书进行。待标本收集完毕,取出标本EP 管,常温下融化,为避免标本放置时间较长影响检测结果,每次检测20 个标本,同时检测3 种炎症因子。使用前室温平衡20 min,设计标准品孔、样本孔、空白孔,标准品孔按次序各加不同浓度的标准品50 μL ;样本孔先加待测样本10 μL,再加样本稀释液40 μL,空白孔不加;除空白孔外,标准品孔和样本孔中每孔加辣根过氧化物酶(HRP)标记的检测抗体100 μL,用封板膜封住反应孔,37℃恒温箱温育60 min ;弃去液体,吸水纸上拍干,每孔加满洗涤液,静置1 min,甩去洗涤液,吸水纸拍干,如此重复5 次,洗涤液用洗涤缓冲液与蒸馏水1∶19 配制;每孔加入底物A、B 各50 μL,37℃避光孵育15 min。每孔加入终止液50 μL,15 min 内,在450 nm 波长处测定各孔的光密度值,未用完试剂盒置于4℃冰箱保存,待下次检测时使用,所有标本在3 d 内检测完毕。以上操作均根据试剂盒说明书进行。

1.4 统计学处理

采用SPSS 17.0 软件对数据进行统计学分析。计量资料采用Pearson 相关分析,分别分析引流液、血液中炎症因子浓度与VAS 评分相关性,CRP、ESR 与血液、引流液中炎症因子相关性。检验水准α=0.05。

2 结 果

术后1 d 引流液中炎症因子IL-1β 浓度与反应性疼痛VAS 评分有弱的相关性;IL-6、TNF-α 浓度与反应性疼痛VAS 评分无相关性。术后2 d 及3 d 各炎症因子浓度均与疼痛VAS 评分有相关性(P < 0.05,表 12)。引流液中炎症因子浓度与ESR、CRP 无相关性(表 3)。血液中各炎症因子浓度与腰椎术后疼痛VAS 评分均无相关性(P > 0.05,表 3)。

3 讨 论

目前对于TNF-α 引起腰痛的机制主要以下几点。退变椎间盘中高浓度的TNF-α 对中性粒细胞和单核细胞有趋化作用,新生血管长入增加了炎症因子的刺激作用,手术过程中,微环境的破坏可促进炎症细胞浸润,刺激炎症因子增加。TNF-α 作用于血管内皮细胞,诱导血管内皮细胞产生其他炎症介质,与IL-1β 等共同引起局部的炎症反应,产生疼痛[10]。椎间盘髓核细胞可以产生TNF-α,导致神经纤维内膜水肿、腰背神经根疼痛过敏[11]。研究发现髓核组织可减慢神经传导速度,而TNF-α 诱导的神经传导速度减慢则比髓核尤甚,并认为髓核细胞分泌的TNF-α 与神经功能障碍和髓核本身导致的疼痛有关[12]。IL-1β 是一种强有力的致痛性炎症因子,它是IL-1 细胞外主要亚型,TNF-α 致痛作用除增强腰背神经根敏感性[11],主要是与IL-1β 和IL-6[13]一起通过刺激突出椎间盘组织释放前列腺素E2(PGE2)作用于神经根发挥致痛作用,主要表现为对脊神经的痛性刺激,受其刺激后脊髓背根神经节敏感性显著上升,同时增强TNF-α 的敏感性[14]。IL-6 是IL 家族中另外一种重要的炎症促进剂,可在椎间盘内自然产生,IL-1β 可刺激IL-6 显著增加[15],从而刺激炎症细胞聚集,激活炎症介质的释放,使背神经节敏感性上升,导致腰背痛和坐骨神经痛。以上炎症因子(IL-1β、IL-6、TNF-α)主要通过椎间盘内生物学作用及刺激纤维环内感觉神经末梢产生疼痛,所以与血液中炎症因子无明显关系。上述基础研究文献均证明炎症因子在慢性退变的椎间盘引起疼痛刺激。

一些体外实验和组织标本检测也证实腰椎椎间盘髓核内存在丰富的炎症因子,如IL-1β,IL-6,TNF-α 等[16]。在出现椎间盘退变及突出时,炎症因子的释放接触神经根及窦椎神经可以导致下腰痛和根性放射痛,这被认为是椎间盘病变引起症状的原因之一[16, 17, 18, 19]。正常的椎间盘是一个封闭的组织结构,髓核内的炎症因子不会释放出来与神经结构接触,但因椎间盘病变、损伤或手术致纤维环破裂时,内部的炎症因子得以释放,出现疼痛症状[20]。本组在做腰椎椎间盘切除手术时,术后患者常可见到短期的非手术切口部位的疼痛,不能用神经压迫及损伤来解释,推测其为炎症因子导致的反应性疼痛。如果是炎症反应性疼痛,就可在临床上进一步证实炎症因子在椎间盘病变和损伤中的作用。为证实炎症因子在椎间盘病变中所起的作用,有学者采集椎间盘标本进行椎间盘内炎症因子检测,但该方法不能在临床上证实炎症因子就是致痛的原因[16],而检测术后伤口引流液中的炎症因子水平则能证明术后椎间盘中所含的炎症因子因椎间盘纤维环完整封闭作用的破坏导致炎症因子的释出。基于此种假设,本研究检测并分析了引流液中的炎症因子水平是否与术后反应性疼痛有相关性。但术后引流液中炎症因子水平的改变如果是全身反应的局部表现,则不能说明炎症因子与反应性疼痛的相关性。因此,本研究同时检测了血液中的炎症因子水平,与反应性疼痛也进行了相关性比较,发现血液中的炎症因子水平与反应性疼痛不相关,同时,作为反映全身炎症活动性指标的ESR 和CRP 也进行了检测,未发现与反应性疼痛有相关性。进一步说明腰椎椎间盘术后引流液中的炎症因子升高不是全身反应的一部分,而是局部炎症因子浓度的增加所致,而炎症因子在伤口中浓度的增加有可能是椎间盘破坏以后释放出的炎症因子所致。本研究对于是否椎间盘释放炎症因子导致局部炎症因子浓度增高无法给出直接的证据,还有待进一步的研究。

本研究通过术后短期连续观察,并通过评价炎症因子浓度与VAS 评分相关性发现:术后当日,在VAS 评分≥ 3 分的患者中,引流液中炎症因子IL-6、TNF-α 浓度变化与VAS 评分无明显相关性,IL-1β 在术后第1 天与反应性疼痛VAS 评分有弱相关性,说明反应性疼痛出现的时间及炎症因子的升高不一定正好开始于术后2 d。术后2 d 和3 d 引流液炎症因子浓度与反应性疼痛呈正相关,可能原因是引流液炎症因子基本在术后2 d 开始升高,这与临床观察一致,患者往往术后2 d 开始出现明显的反应性疼痛,但因术后常规引流管为24 ~ 72 h 拔除,因此,3 d 以后伤口内炎症因子浓度的变化无法得到,也是本研究的缺憾。引流液总量与炎症因子浓度及VAS 评分均无相关性,说明引流液总量对炎症因子致痛作用无影响,而炎症因子的浓度与术后反应性疼痛相关。本研究中,术后外周血炎症因子浓度、ESR 及CRP 与腰椎术后引流液中炎症因子浓度基本无相关性,虽术后1 d IL-6 与ESR 也出现弱的正相关,但多数情况下无相关性,说明血液中炎症因子对腰椎术后反应性疼痛亦无影响。本研究显示,并不是每位患者在腰椎术后都产生反应性疼痛,本研究中40 例患者中有13 例出现腰椎术后反应性疼痛,而其他病例未出现,可能因为本研究剔除了反应性疼痛< 3分的病例,而患者术后疼痛阈值的不同所导致,并不能排除其余病例可能出现的反应性疼痛。有文献报道下腰痛与外层纤维环内感觉神经末梢密度有密切关系[21],该现象也可能与此有关。最新的研究显示,不仅炎症因子影响腰痛及坐骨神经痛的程度,而且也和炎症因子的相互作用和失衡有关系[22, 23]

本研究的临床意义在于,既往文献报道均为体外实验证明椎间盘病变导致的腰痛。本实验在临床上证明炎症因子与腰痛有明显的相关性,而不只是单纯的机械性慢性劳损的原因。在临床上常出现反应性疼痛,如出现这样的情况,基本可以排除手术的问题,而考虑为炎症因子所致,从而可进行相应的抗炎治疗。

4 结 论

腰椎术后反应性疼痛与术后引流液中IL-1β,IL-6,TNF-α 水平显著相关。腰椎术后反应性疼痛与术后炎症因子的局部释放增加可能有密切的关系。
