椎间盘退行性变(IDD)是诸多脊柱退行性疾病的病理基础,尽管其发生机制仍没有完全清楚,但是普遍认为椎间盘细胞外基质(ECM)降解代谢的显著增加是重要原因之一。在过去10年中,通过利用人体样本与动物模型对椎间盘ECM降解的研究已经取得了巨大的进步,其中认为降解ECM(如聚蛋白聚糖)的关键酶——含血小板凝血酶敏感蛋白的解聚蛋白样金属蛋白酶(ADAMTS)家族在此过程中发挥了重要的作用[1-2],因而ADAMTS家族在IDD中的表达和调控也成为相关领域的研究焦点[3]。本文就目前ADAMTS家族在IDD中的相关研究进展作一综述,以期进一步完善对IDD分子机制的理解。
1 ADAMTS的结构、分类及生物学特征ADAMTS是继基质金属蛋白酶(MMPs)后被发现的一种新型锌离子依赖的分泌型金属蛋白酶。尽管与MMPs同属金属蛋白酶家族,但是相比于MMPs,人们对ADAMTS却知之甚少。ADAMTS在哺乳动物和无脊椎动物体内广泛存在,其可通过特殊蛋白区域与细胞表面及ECM结合,在关节炎、血管生成、动脉粥样硬化、炎症反应、肿瘤的生长和转移等病理生理过程中具有重要作用,和人类的多种疾病密切相关[4]。ADAMTS家族的结构较为相似,从N端至C端依次为信号肽、前结构域、一个带锌离子结合位点的金属蛋白酶催化结构域、解聚蛋白样结构域、半胱氨酸富集区、半胱氨酸间隔区、凝血酶敏感蛋白(TSP)重复序列。目前,已经发现了近20种ADAMTS。ADAMTS根据其结构和功能可分为4类:① 透凝蛋白聚糖酶,ADAMTS-1、4、5、8、9、15、20;② 血管假性血友病因子,ADAMTS-13;③ 原骨胶原N端肽酶,ADAMTS-2、3、14;④ 暂时未知其功能的ADAMTS,包括ADAMTS-6、7、10、12、16、17、18、19[3]。
聚蛋白聚糖占人类椎间盘髓核组织干质量的50%以上,是髓核ECM最重要的组成成分之一。聚蛋白聚糖通过连接蛋白结合透明质酸,从而形成带负电及亲水性的聚合体[5]。该聚合体又与胶原蛋白结合,形成可包含大量水分的网状结构,使正常椎间盘处于水合状态,从而使椎间盘能够传导相邻椎体间的压力,维持椎间高度,并承载负荷以及分散应力[6-7]。研究表明,ADAMTS-1、4、5、8、9、15、18均可降解聚蛋白聚糖的活性,其中以ADAMTS-4、5对聚蛋白聚糖的降解作用最为强烈[8-11]。聚蛋白聚糖结构域中有2个主要的水解切割位点,其中Glu373-Ala374位点为ADAMTS的作用位点[12],另一个位于Asn341-Phe342的切割位点则为MMPs作用位点[13]。而ADAMTS-4,5除对以上2个主要的位点均具有酶切作用外,还可以通过Glu1667-G1u1668、Glu1480-Glu1481、Glu1871-Glu1872、Glu1771-Glu1772等位点对聚蛋白聚糖发挥降解作用。尽管有研究表明,通过多个酶切位点对聚蛋白聚糖进行降解的效率要比单纯的通过Glu373-Ala374位点高,但普遍认为ADAMTS主要还是通过Glu373-Ala374酶切位点来降解聚蛋白聚糖[14-15]。此外,ADAMTS间隔区与纤维连接蛋白结合后会抑制聚蛋白聚糖的水解[16]。Sandy[17]认为,相比于MMPs,ADAMTS所调节的聚蛋白聚糖分解中糖胺聚糖(GAG)的损耗与软骨的降解更具有破坏性。而进一步的研究亦表明,在正常以及骨关节炎中的关节软骨退变中,聚蛋白聚糖的降解主要依靠ADAMTS的作用,而非MMPs[18]。但是,在机械应力下诱导的IDD模型中,正常椎间盘中ADAMTS的表达被显著升高的MMPs所取代[19]。
2 椎间盘中ADAMTS的表达ADAMTS-1、4、5、9、15的mRNA和蛋白已被证明存在于正常人类椎间盘组织中,表明这些ADAMTS在椎间盘正常的生理过程中具有一定的作用[20]。在退变的椎间盘中没有检测到ADAMTS-9的表达,而ADAMTS-5是否在退变过程中表达,目前仍有分歧[20]。在正常髓核和纤维环中ADAMTS mRNA水平的表达没有显著的差异,但是髓核和内层纤维环中ADAMTS蛋白水平的表达要更高于外侧纤维环中的表达[20]。当IDD时,ADAMTS-1、4、5、15 mRNA表达显著升高[21],并且ADAMTS-4在纤维环中的表达超过了髓核中的表达,而ADAMTS-1、5、15 mRNA的表达在髓核中较多;在蛋白水平,ADAMTS-1、4、5、9、15在髓核与内层纤维环中均有升高,其中ADAMTS-4在髓核中升高的水平较内层纤维环中更为显著[20-21]。此外,Patel等[22]比较了轻度与重度退变椎间盘组织中蛋白表达的差异。他们发现,髓核与纤维环中ADAMTS-4的表达随着退变的进展而增加,而ADAMTS-5的表达随退变程度的改变并不明显。这些结果说明,IDD过程中ADAMTS的表达模式仍然不清楚,并且存在较大的争议。应用基因敲除小鼠模型的动物研究已经证实,在骨关节炎中ADAMTS-5是主要降解聚蛋白聚糖的工作酶[23]。相对的,在IDD领域中,有研究比较了小鼠IDD模型中ADAMTS与MMPs对聚蛋白聚糖的降解作用,结果显示,在IDD中ADAMTS活性减弱,而MMPs所调节的基质降解却显著增加[19]。ADAMTS与MMPs在ECM降解代谢中的相对重要性长久以来一直存在争论。在人类椎间盘中已经分别检测到了MMPs与ADAMTS产生的聚蛋白聚糖碎片,这些碎片在退变样本中的存在显著多于正常样本[24-26];其中MMPs所产生的聚蛋白聚糖碎片随着老化而逐渐增加,但与退变的严重程度无相关性,而ADAMTS所产生的聚蛋白聚糖碎片随着老化而逐渐减少,并且其水平亦与退变的严重程度无关。此外,已有研究证明,IDD中MMPs所产生的聚蛋白聚糖碎片显著多于ADAMTS所产生的[27]。因此,可以证明,椎间盘中ADAMTS的工作模式与作用是极其复杂的,相关的机制仍需要进一步的研究,以评估其在椎间盘生理与退变过程中的作用。
3 ADAMTS在IDD中表达的调控在椎间盘中,ADAMTS的表达被多种因素所调控,如机械应力、炎性反应、氧化应激以及吸烟等环境因素。在IDD的过程中,这些因素相互作用,从而启动并且促进ADAMTS所介导的ECM降解,最终导致椎间盘结构破坏。
3.1 机械应力在ADAMTS家族中,ADAMTS-4、5被认为是作用最强的聚蛋白聚糖酶。现有的研究已经表明,椎间盘中ADAMTS-4的表达受机械应力刺激的影响,并且该作用与机械应力的强度、频率及持续时间相关[28]。Maclean等[29]在活体模型中对椎间盘施加动态压力,当压力处于中等水平(1.0 MPa)时,髓核与纤维环中ADAMTS-4基因表达显著增加,而当压力水平较低(0.2 MPa)时,该基因表达不受影响;此外,该研究还发现当压力维持在1.0 MPa时,施加压力频率的改变对ADAMTS-4的表达没有明显的影响。同一个团队的另一项研究发现,在椎间盘受到动态压力2 h左右时,髓核中ADAMTS-4的表达达到峰值,而纤维环中,ADAMTS-4的峰值出现在4 h左右[30]。Barbir等[31]的研究还证实,躯体的旋转对椎间盘中ADAMTS-4的表达也有影响,当旋转角度达到30°时,ADAMTS-4的表达显著增高。此外,ADAMTS-7和ADAMTS-12也被证实受机械应力的调控[32],研究发现,对鼠尾椎间盘施加机械应力的第一个18 h,髓核组织中ADAMTS-7和ADAMTS-12的表达几乎没有变化,但18 h后ADAMTS-7和ADAMTS-12的表达均显著升高,ADAMTS-7表达的峰值出现于第1天,ADAMTS-12的表达峰值出现于第7天。该结果表明,椎间盘组织中ADAMTS的表达对压力量级的变化极其敏感,并且作用频率以及作用的持续时间亦对ADAMTS的表达具有调控作用,从而成为IDD发生的助力。
3.2 炎性反应炎性反应亦与IDD息息相关。有报道证实,在人退变的椎间盘中,炎症因子如肿瘤坏死因子(TNF)-α,白介素(IL)-1β等显著升高[33-34]。有研究表明,TNF-α的表达与老化以及组织学退变等级具有紧密的关系[33]。在IDD的动物模型中,纤维环针刺模型是一种通过纤维环损伤造成急性炎性反应导致IDD的经典模型,在受损的椎间盘组织附近可以检测到P38阳性的细胞大量分泌TNF-α和IL-1β,ADAMTS-4、5的表达也显著升高[35-36]。此外,IL-1β受体拮抗剂的使用可以显著延缓IDD的进度,并且ADAMTS-4免疫染色阳性的细胞数目也显著下降[37]。体外研究也证实了这一结果,使用TNF-α和IL-1β干预大鼠髓核细胞,ADAMTS-4、5在mRNA和蛋白水平的表达均有显著的升高[38-40]。Tian等[41]的研究进一步证实TNF-α或IL-1β所介导的ADAMTS-4表达增高起始于转录水平,并且上游激酶MAPK与转录因子NF-κB启动该基因的转录。Wang等[42]的研究证实TNF-α和IL-1β在转录水平诱导了ADAMTS-5的表达,在该过程中SDC-4的表达是介导该转录的关键;此外,他们还发现NF-κB对SDC-4的表达具有重要调控作用。Fujita等[43]的研究发现,PHD3在炎症因子诱导ADAMTS-5的表达过程中具有重要作用;同时他们也认为NF-κB参与了这一过程。转录因子NF-κB是经典的炎性反应通路,最近的研究再一次证实在人髓核细胞中NF-κB的抑制可导致ADAMTS-4、5表达的下降,从而缓解ECM的降解[44-45]。这些研究结果表明,TNF-α和IL-1β通过MAPK和NF-κB等通路对椎间盘组织退变过程中ECM的降解产生重要作用,而炎性反应的抑制可能可以作为IDD分子治疗最有希望的靶点之一。
3.3 氧化应激及吸烟等环境因素此外,也有相当的证据表明氧化应激也是导致IDD中ECM降解的重要因素之一。免疫形态学分析显示,相比于年轻患者,老龄患者退变的椎间盘中氧化应激蛋白的指标更高,并且氧化应激信号通路关键转录因子NF-κB在老龄患者中也显著增高[46]。正常的髓核处于低氧环境中,退变过程中血管长入等外在因素均会使髓核细胞中活性氧(ROS)增多,从而导致氧化应激增强。ROS不仅能够损伤髓核细胞DNA导致细胞凋亡,还能够激活MAPK与NF-κB通路加速IDD[47]。研究显示,将人类椎间盘细胞暴露于常氧状态(20% O2)下,MMPs和ADAMTS-4、5的表达会显著增加,这意味着氧化应激能够对下游的降解基因发挥调控作用[47]。此外,使用烟草提取物干预椎间盘细胞可以导致MMPS和ADAMTS的显著高表达,而研究证实,这也与烟草提取物中大量的氧化剂所致的氧化应激反应有关[48]。
4 总结与展望目前关于IDD的治疗主要在于改善其症状,而非基础病因。ADAMTS家族在IDD过程中能够降解ECM中的重要组成成分—聚蛋白聚糖,从而导致椎间盘最终丧失功能。早期减少IDD中的分解代谢可能是比较可行的治疗IDD的方法,因为退变的椎间盘缺乏营养并且微环境改变严重,试图通过增加合成代谢来促进椎间盘恢复更困难。目前,抑制ADAMTS活性已被证实在延缓骨关节炎的病程中效果良好。因此,抑制ADAMTS的表达不失为一种治疗IDD的新策略。但是,目前ADAMTS家族在IDD中的具体作用仍未完全研究透彻,而且特异性的治疗药物也面临给药途径、椎间盘内部微环境、药物代谢途径等挑战。深入研究ADAMTS家族的作用与具体调控机制,尤其是在IDD中发挥核心作用的ADAMTS成员的作用机制,可为未来IDD的生物治疗提供一个新思路。
