      目的 探讨侧路非同轴椎间盘镜在腰椎椎间盘切吸术的应用价值。方法 141例患者(183个节段)经临床确诊为腰椎椎间盘突出症,其中男76例,女65例;平均年龄为42.5岁,病史为3 d~130个月;L4/L5 79例,L5/S1 83例,L3/L4 21例,平均住院6.8 d。手术方法同经皮穿自动腰椎椎间盘切除术(automated percutaneous lumber diskectomy,APLD),置入工作套管后在非同轴椎间盘镜监视下嵌夹、切吸椎间盘髓核组织。术后3、6、12个月时随访。结果 141例患者手术均获成功,无并发症发生,术中嵌取、切吸髓核组织1.0~3.2 g,复查CT值减低15~20 HU,按MacNab腰腿痛手术评价标准判定,有效率为92.1%。结论 在APLD中应用椎间盘镜可以减少手术的盲目性,减少医生及患者的X线照射剂量;针对性的嵌夹、切吸椎间盘髓核组织对防止术后失稳性腰痛、提高疗效有重要的意义。

Abstract:
      Objective To explore the value of noncoaxial diskoscope in lateral lumbar discectomy. Methods Totally 141 patients (183 segments) with lumbar disc herniation were enrolled in this study. There were 76 males and 65 females aged from 18 to 78 years (average 42.5 years), with the history from 3 d to 130 months and the average hospital stay of 6.8 d. There were 79 cases in L4/L5, 83 in L5/S1, and 21 in L3/L4. The surgical procedure was similar to the automated percutaneous lumber diskectomy (APLD). After working casing was placed, the nucleus gelatinosus were griped and clipped under noncoaxial diskoscope. The patients were followed up 3, 6 and 12 months after the operation. Results The operation was successfully completed in all the 141 patients without complications. The clipped nucleus gelatinosus weighed 1.0-3.2 g. The average CT value was decreased by 15-20 HU. The effective rate was 92.1% according to MacNab waist and leg surgery evaluation standard. No serious complications occurred. Conclusion The use of endoscope in APLD can reduce blind operation and the X-ray irradiation dose for surgeons and patients. It is helpful to prevent low back pain caused by postoperative instability and improve the therapeutic efficacy by targeted excision of nucleus pulposus.
